SOCIAL WORKER/MENTAL HEALTH PROFESSIONAL (Float Pool Position)

Job Title

Completes psychosocial assessments utilizing the data toward formulating a diagnostic impression of the patient and family’s social and emotional functioning, and assists in developing and implementing a Treatment and Discharge Plan. Maintains knowledge of and adheres to federal and state regulatory guidelines and timelines in performing duties. Interviews and interacts with patients and when indicated, their advocates, to obtain mental status and psychiatric and social histories relevant to their current emotional/behavioral problem(s). Uses current evidence-based knowledge and skills to assess psychiatric emergencies and to provide sound, professional care and crisis intervention services to patients. Collaborates with outside facilities referring patients to CHMC to collect documentation, review and present to on call provider if beds are available. Facilitates therapeutic group intervention Provides individual therapy treatment to patients Participates in multi-disciplinary team meetings.
