Telehealth Nurse Practitioner - Multi-State Licensed

MUST HAVE AN ACTIVE LICENSE (APRN) IN ANY OF THESE STATES Texas, Illinois, Connecticut, Indiana, New Hampshire, California, and Georgia. OVERVIEW We are currently seeking a skilled and compassionate Telehealth Nurse Practitioner licensed in multiple U.S. states to join our growing virtual care team. This role plays a vital part in providing services such as medication therapy management, chronic care support, and care coordination in close collaboration with partnering health centers. RESPONSIBILITIES Conduct virtual assessments and consultations, reviewing patients’ medical histories, medication regimens, and current health status. Provide patient counseling and education, delivering guidance on preventive care, chronic disease management, lifestyle modifications, and medication adherence. Coordinate care and make referrals by working with health center care teams, specialists, and community-based providers to ensure seamless coordination of care, including necessary referrals and follow-ups. Maintain documentation by accurately documenting patient encounters and care plans in the electronic medical record system in a timely and compliant manner. REQUIREMENTS Proficiency with EMR systems and virtual care platforms. Background in primary care, chronic care, and medication reviews. Strong communication skills and ability to work independently and within a collaborative care team. Flexibility to provide care outside standard business hours occasionally.
