As a Licensed Practical Nurse (LPN), you will provide supervision of staff and safeguard the health, safety, and welfare of all residents under your care. We're looking for a dedicated individual with a passion for delivering high-quality care to join our Skilled Nursing team.• Deliver safe, timely, and effective nursing care to residents under the direction of an RN or physician• Monitor patients' health, administer medications, and provide wound care and other treatments• Collaborate with interdisciplinary teams to develop and update individualized care plans• Support residents and their families with education, comfort, and reassurance• Maintain accurate documentation and comply with regulatory standards• Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team• Observes safety policies and reports any safety concerns• Demonstrates respect of resident rights and dignity• Fulfills the role of charge nurse when assigned• Supervise and support CNAs in the delivery of care• Maintain a clean, safe, and therapeutic environment for residents• Ensure compliance with infection control, safety, and HIPAA policies• Participate in quality assurance activities and staff meetings• Active LPN license in Maryland or Multistate• Experience in long-term care or skilled nursing preferred – new grads welcome!• Strong clinical skills, attention to detail, and a heart for service• Ability to thrive in a fast-paced, team-oriented environment• Strong clinical skills and attention to detail• Excellent communication and interpersonal skillsAt South Mountain Rehab Center, we believe our people are our greatest asset.
