Diabetes EducatorHourly Pay Range: $35.16 - $54.50 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.Position HighlightsLocation: 100 Spalding Drive, Naperville, IL and 130 N Weber Rd, Bolingbrook, ILFull Time: 40 hours / weekHours: Varies Monday-Friday, 7:30 AM - 6:00 PMRequired Travel: This role includes some travel to other locations and requires flexible hours to meet operational needs.What You Will DoDemonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served.Assesses patient's condition based upon observation and/or interview of patient, physician's diagnosis, laboratory results and any other pertinent information.Develops and implements a plan of care based on assessment diabetes self-management needs to patients and family members.Provides instruction on insulin and administration of insulin through subcutaneous injection utilizing appropriate educational materials.Provides instruction on programming insulin pumps and pattern management utilizing appropriate educational materials.
