On-Call Home Health & Hospice Registered Nurse (RN)Full Time 36 Hours - Friday 5 PM to Monday 8 AMLocation: 2 Positions Available: 1 South Team (Newton to Kingston areas), 1 North Team (495 Corridor Marlboro to Haverhill)Reports To: Executive DirectorSalary Range: $15 Hour on call base pay & paid per visit (revisits $90, admission $165)Sign on Bonus: $2,500 paid in 2 installments 50% in 6 months, 50% in 12 months of start date.Home is Where the Heart Is! Whether you're looking for help to live independently, manage your health, receive home care services, or find support during life's most challenging times, we're here for you every step of the way.Position SummaryThe On-Call RN is responsible for providing skilled nursing care and support to patients and their families during after-hours, weekends, and holidays. This role ensures patients receive timely and compassionate care by responding to calls, triaging symptoms, and making visits as needed during the on-call shift.Key ResponsibilitiesRespond promptly to after-hours calls from patients, families, or facilities.Provide phone triage, assess symptoms, and determine the appropriate level of intervention.Make home or facility visits as necessary to provide hands-on nursing care.Communicate changes in patient condition to the interdisciplinary team.Provide emotional support and guidance to patients and families during crisis situations.Document all visits, calls, and care provided in accordance with agency policy and home health regulations.Collaborate with the on-call manager and case managers to ensure continuity of care.Participate in on-call rotation and remain available during assigned shifts.Follow all home health policies, procedures, and safety guidelines.QualificationsCurrent and unrestricted Registered Nurse (RN) license in the practicing state.Minimum of 1 year of recent home health experience preferred.Excellent clinical assessment and critical thinking skills.Strong communication, interpersonal, and organizational skills.Ability to work independently in a home or facility setting.Valid driver's license, reliable transportation, and proof of auto insurance.Proficient in electronic medical record (EMR) systems and documentation.Work ScheduleThis is an on-call position covering weekends (Friday 5 PM - 8 AM, Sat & Sun 8 AM - 8 PM) and/or holidays as assigned.Work EnvironmentPatient homes, assisted living centers.Requires travel within a defined service area.
