Occupational Therapist

Centra Healthcare Solutions

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment.

Responsibilities

Assess referred patients requiring rehabilitation services, including direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation.
Establish and administer treatment programs with specific goals determined by the patient's capacity and tolerance under physician direction.
Set realistic and achievable goals for patients, document and record patient condition.
Educate patients and families on appropriate occupational therapy methods.
Collaboration with all disciplines to plan and evaluate team goals for each patient.

Requirements

Graduation from an accredited school.
Current state license as an Occupational Therapist / OT or actively pursuing licensure.
Current CPR certification.
