Physical Therapist - Travel Contract

Minimum Requirements
Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Indiana PT license required to start the assignment.
BLS certification from AHA or ARC may be required.
Experience in outpatient orthopedics or general outpatient physical therapy preferred.
Demonstrated ability to perform comprehensive evaluations, write functional treatment plans, and document per payer and clinic guidelines.
Experience supervising or mentoring students or support staff preferred.
Maintain professional licensure and complete state-mandated continuing education.

Assignment Details
Facility type: Outpatient clinic.
Shift pattern: 5x8 schedule.
Duration: 13 week assignment.
Floating: Candidate may be required to float to other clinic locations as needed.
