Occupational Therapist (OT) – Home Health

Location: Riverside, CA
Setting: Home Health
Schedule: Monday–Friday with one weekend day on-call

Job Summary

Seeking a compassionate and motivated Occupational Therapist (OT) for a Home Health assignment in Riverside, CA. This role offers the opportunity to provide individualized therapy services to patients in their homes while promoting independence and improving quality of life.

Required Qualifications

• Active California OT License or pending California license (application must be submitted prior to submission)
• OASIS experience required
• Ability to work independently and manage a home health caseload

Preferred Qualifications

• Home Health experience preferred
• Candidates without Home Health experience will be considered

Key Responsibilities Perform occupational therapy evaluations and develop individualized treatment plans Complete OASIS assessments and required documentation Educate patients and caregivers on therapeutic interventions and safety strategies Collaborate with interdisciplinary healthcare teams to support optimal patient outcomes Monitor patient progress and adjust treatment plans as needed
