LPN Hospice Crisis Care

Addus Family Company is seeking an LPN for Hospice Crisis Care to provide intensive bedside support to patients experiencing acute changes or end-of-life symptoms in their homes or facilities. In this mission-driven, patient-focused role, you’ll perform assessments, manage pain and complex symptoms, administer medications, and implement crisis care plans in coordination with the RN case manager and hospice team. You’ll educate and support families, document care in the EMR, and ensure compliance with hospice standards while working collaboratively with an interdisciplinary team.
