Location: Albany Region, New York (Hoosick Falls, Cambridge, and Troy)

Position Overview:
We are seeking an experienced Nurse Practitioner to provide comprehensive primary care services to residents in assisted living communities throughout the Albany Region of New York. This position offers a high level of autonomy, flexible scheduling options, and the opportunity to build meaningful relationships with older adult patients while delivering high-quality, patient-centered care.
The ideal candidate has strong primary care experience, enjoys working with the geriatric population, and is comfortable practicing independently in an assisted living setting.
Key Responsibilities:

• Provide comprehensive primary care services to assisted living residents
  
  
• Assess, diagnose, and treat acute and chronic medical conditions
  
  
• Perform routine health assessments and preventative care services
  
  
• Order, review, and interpret laboratory and diagnostic testing
  
  
• Prescribe medications and develop individualized treatment plans
  
  
• Coordinate care with facility staff, specialists, caregivers, and family members
  
  
• Maintain accurate and timely documentation within the electronic medical record (EMR)
  
  
• Promote patient wellness, safety, and quality of life
  
  

• Active New York State Nurse Practitioner license
  
  
• Active DEA registration
  
  
• Currently enrolled and credentialed with both Medicare and Medicaid
  
  
• Ability to practice independently in accordance with New York State regulations
  
  
• Minimum of 3,600 clinical practice hours as required by New York State
  
  
• Minimum of 2 years of primary care experience
  
  
• Strong clinical judgment and patient assessment skills
  
  

• Experience caring for geriatric patients
  
  
• Assisted living, long-term care, skilled nursing, or post-acute care experience
  
  
• Strong communication and interdisciplinary collaboration skills
