Our in-house therapy model allows therapists to provide patient-centered care without the limitations of third-party contract staffing.Professional Growth OpportunitiesWe are committed to investing in our clinicians through ongoing education, mentorship, and development opportunities that support long-term career success.Key ResponsibilitiesAs a Physical Therapist at Canyon Springs Post Acute, you will play a vital role in helping residents regain strength, mobility, and independence.Clinical Duties Include:Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals and clinical needsProvide skilled, evidence-based physical therapy treatments in accordance with the California State Practice ActSupervise Physical Therapist Assistants (PTAs) and support Restorative Nursing Aides to ensure continuity of careMaintain timely and accurate documentation, including daily treatment notes, progress reports, and recertificationsCollaborate closely with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as neededEducate residents and families on mobility safety, home exercise programs, and discharge planningRecommend and coordinate durable medical equipment (DME) such as walkers, wheelchairs, and braces to support successful recoveryQualificationsCurrent and unrestricted California Physical Therapist (PT) license requiredStrong clinical judgment and documentation skillsExcellent communication and interpersonal abilitiesAbility to motivate and engage residents throughout the rehabilitation processPositive, team-oriented attitude with a passion for helping others regain independenceAt Canyon Springs Post Acute, we are committed to delivering exceptional rehabilitation services in a supportive and rewarding environment.
