What You'll Do
- Screen, evaluate, and treat patients with physical functioning disorders and/or wound care needs.
- Develop and implement individualized plan(s) of care consistent with regulatory and clinical standards.
- Use standardized assessments and evidence-based practice to guide therapy interventions.
- Provide therapeutic services including manual techniques, ambulation, therapeutic exercises, modalities, assistive/supportive device use, and wound-related care.
- Monitor patient progress, adjust treatment plans, and document outcomes in the electronic record.
- Communicate regularly with the clinical team and leadership regarding progress, barriers, and care plans.
- Prepare discharge summaries and complete required documentation and treatment minute tracking.
- Participate in required facility/company meetings and trainings.
What You Need
Required- Valid Physical Therapist (PT) license (per state requirements)
- Adherence to APTA standards and state practice act, code of conduct, and ethics
- Ability to document clinically required information in an electronic documentation system
- Ability to support and/or supervise appropriate rehabilitation personnel as required
- Experience in Long Term Care (LTC) or skilled nursing/rehab settings (if applicable based on your background)
- Familiarity with wound care and rehabilitation outcome evaluation

This position is with a leading facility in Haverhill, Massachusetts, supporting patients in a Long Term Care (LTC) environment. You'll work as part of an interdisciplinary team, providing goal-directed physical therapy services, tracking outcomes, and supporting patients through treatment planning, progress updates, and discharge documentation.
