Physical Therapist

MUSC Health at Home by BAYADA has an immediate opening for a Per Diem Physical Therapist, Home Health to provide home health visits for our office in Summerville, SC and surrounding areas. As a home health care PT, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. Responsibilities Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns, appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, weight‑bearing balance, and other interventions. Continuously assess and revise the Physical Therapy care plan and collaborate with nursing in the multidisciplinary care plan, as appropriate. Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote client health, safety, and independent living. Accurately document observations, interventions, and evaluations pertaining to client care management and services, utilizing a state‑of‑the‑art touch‑pad tablet. Qualifications A current South Carolina Physical Therapist license. A minimum of one year of recent work experience as a Physical Therapist. Graduation from an APTA‑approved program or a program approved by the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by a school transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills.
