Licensed Practical Nurse (LPN)

Signature HealthCARE is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team, where you will play a key role in delivering high-quality, resident-centered care. As an LPN with Signature HealthCARE, you will provide direct nursing care to residents and patients in a long term care and skilled nursing environment, collaborating closely with RNs, physicians, therapists, and other interdisciplinary team members to support optimal health outcomes. You will perform comprehensive and focused nursing observations, obtain and document vital signs, administer medications and treatments according to physician orders and facility policies, and assist with wound care, catheter care, G-tube care, and other clinical procedures within the LPN scope of practice. You will be responsible for accurate and timely documentation in the electronic health record, ensuring your notes reflect changes in condition, responses to treatment, and communication with families and providers. In this role, you will help develop, implement, and update individualized care plans, actively participate in care conferences, and communicate effectively with residents, families, and the care team to promote understanding, safety, and satisfaction. You will supervise and direct certified nursing assistants and other unlicensed staff in the delivery of daily care, offering guidance, coaching, and real time feedback to ensure adherence to care plans, infection control practices, and resident rights. A key aspect of this position is your commitment to safety and quality improvement: you will monitor residents for changes in condition, report and respond promptly to emergencies, support fall prevention and skin integrity initiatives, and participate in audits, in services, and performance improvement projects that align with Signature HealthCARE’s culture of continuous improvement and innovation. You will champion resident dignity, autonomy, and quality of life, assisting with pain management, behavioral support, and end-of-life care when needed, while maintaining strict confidentiality and compliance with HIPAA and all applicable state and federal regulations.
