Admissions Registered Nurse (RN) – Home Health Visits

Full-Time | Monmouth, Middlesex, Union & Somerset Counties, NJ

BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time opportunity with our UCV office , providing home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients throughout Monmouth, Middlesex, Union, and Somerset Counties, NJ .

As an Admissions RN, you'll play an important role in welcoming new clients to BAYADA and setting them up for a successful home health experience. You'll complete comprehensive initial assessments, establish individualized plans of care, and collaborate with our interdisciplinary team to ensure a smooth transition into home health services.

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
• Perform thorough initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of each client's clinical and functional needs.
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and members of the interdisciplinary team.
• Identify client needs and coordinate appropriate nursing, therapy, social work, and other home health services.
• Review and execute physician orders and ensure the plan of care accurately reflects each client's needs and goals.
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.
• Communicate clinical findings and changes in condition to physicians and appropriate members of the BAYADA care team.
• Ensure timely and accurate completion of OASIS assessments and all required admission documentation.
• Accurately document assessments, interventions, education, and care coordination using BAYADA's electronic medical record (EMR) system.
• Help ensure a smooth transition from referral to admission and ongoing home health services.

• Current and unrestricted New Jersey Registered Nurse (RN) license .
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Ability to work independently and effectively manage a field-based schedule.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties .

As an Admissions RN, you're often the first BAYADA clinician a new client and family meets . Your assessment and clinical judgment help establish the foundation for the client's entire home health experience.

You'll have the opportunity to use your nursing expertise in a highly autonomous role while still having the support of an experienced interdisciplinary team. Rather than managing a traditional facility assignment, you'll focus on one client at a time and develop care plans tailored to each client's unique needs, goals, and home environment.

Base Pay, depending upon qualifications, status, and subject to negotiation:

$65.00 - $75.00 per point
