Job Description:

The Registered Nurse in Home Health is responsible for providing nursing care in patients' homes, covering home health and hospice roles with training and support. The RN will manage a case load, seeing each patient once per week, and work alongside an LPN.

Responsibilities:

• Provide nursing care to 5-6 routine home health or hospice patients daily, admitting approximately 2 new patients per day.
• Perform case management duties for 20-25 patients with weekly visits.
• Collaborate with LPN and other care team members to deliver comprehensive care.
• Maintain accurate and timely documentation using the Epic Remote charting system.

Requirements:

• RN with a minimum of 1.5 years of experience in home health and hospice.
• Current BLS certification and valid Driver's License.
• Completion of a nursing education program approved by a board of nursing and South Dakota Board of Nursing RN licensure.
• Proficiency in Oasis, hospice philosophy, infection control practices, wound vacs, wound care, picc line dressing changes, port access, IV infusions, drains; ability to drive at night.
