Description

• Conduct thorough evaluations of patients in either inpatient or outpatient settings and create tailored treatment plans
• Implement physical therapy interventions targeting mobility, strength, balance, and function
• Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercise programs
• Maintain accurate documentation of evaluations, treatments, and progress in compliance with all facility and regulatory standards
• Engage in care team rounds and interprofessional conferences to ensure patient-centered, coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Preferred prior experience in a similar role
• Ability to treat patients with a range of complex medical and mobility challenges
• Excellent communication and teamwork abilities
