Clinical Care Coordinator Location: Terre Haute, IN – Clinical Care Coordinator role at Vituity As a Clinical Care Coordinator, you play a vital role in improving patient experiences by assisting with service authorization, referral coordination, and timely care delivery. Responsibilities Facilitate patient flow in the department Assist with calling patient families Assist with calls to clinics and other hospital facilities Take messages and direct them to the appropriate person in the department Walk service dogs Respond to nurse call lights and inform staff of patient needs Provide comfort items to patients (water, blankets, etc.) Stock non‑medication supplies in the department Assemble urine collection kits Organize nutritional supplies, linen carts, equipment, and utility rooms Notify staff of any faulty equipment Assist with cleaning rooms after each patient is discharged Retrieve wheelchairs and return them to appropriate locations Provide wheelchair assistance Transport specimens to lab if needed Call lab/radiology to inquire about lab results Communicate with RN regarding patients ready for discharge, ensuring all results are completed Notify RN of inpatient bed assignments and assist with patient transport preparations Assist with patient transport Weigh patients Take patient vital signs Reconnect patients to telemetry monitoring and pulse oximetry Perform patient sitter duties Required Experience and Competencies High school diploma or GED required Current BLS/CPR certification (if required by the site) Pre‑med or nursing‑related courses (upper division Anatomy or Physiology a plus) Knowledge of medical terminology preferred Experience with Electronic Medical Records (EMR) preferred Strong writing skills; legibility and spelling proficiency required Solid computer skills; experience with EMR a plus Quick and accurate typing speed Professional demeanor with strong interpersonal and communication skills Excellent organizational skills Knowledge of facility and OSHA safety rules and procedures Knowledge of HIPAA regulations and privacy policies regarding PHI Benefits & Beyond Superior health plan options Dental, Vision, HSA/FSA, Life and AD&D coverage, and more Top‑tier 401(k) retirement savings plans with a 1.20 match up to 6% plus discretionary profit‑sharing (eligible after 18 months) Outstanding paid time off: 4 weeks PTO, paid holidays, and sabbatical options Student loan refinancing discounts Professional and career development program EAP and travel assistance included Wellness program Commuter benefits program Purpose‑driven culture focused on improving lives of patients, communities, and employees Salary range for this role is $16.00 per hour.
