This growth is not only a testament to our models success but the efforts, care, and commitment of our Nurse Practitioners./ppPrimary Responsibilities:/pulliConduct comprehensive in-home assessments, including: ulliPast medical history review, medication reconciliation, vital signs, and physical exam/liliEvidence-based screenings and point-of-care testing (as appropriate)/li/ul/liliManage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions/liliManage members during a short term sub-acute rehabilitation episode/liliEnsure accurate and complete documentation, including ICD 10 conditions/liliCommunicate and collaborate with the interdisciplinary care team/liliConduct advanced illness and advanced care planning conversations/liliProvide patients and caregivers with counseling and education/li/ulpYoull be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in./ppRequired Qualifications:/pulliActive ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care/liliActive, unrestricted licensure in Missouri (or ability to obtain by start date) without current disciplinary actions or practice limitations/liliActive and unrestricted drivers license and access to reliable transportation/liliCurrent, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)/liliAbility to gain a collaborative practice agreement, if applicable in your state/li/ulpPreferred Qualifications:/pulli1+ years of clinical experience as an APC (family, geriatric, or home health preferred)/liliExperience in geriatric medicine, long-term care, senior living or home care setting/liliExperience in meeting the medical needs of patients with complex behavioral, social and/or functional needs/liliUnderstanding of Geriatrics and Chronic Illness/liliUnderstanding of Advanced Illness and end of life discussions/liliProficiency with electronic medical records and technology/li/ulpPhysical Requirements:/pulliAbility to navigate varied environments; ability to sit/stand/kneel as needed to perform duties/li/ulpCompensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
