Case Manager

Job Location: Columbus, OH 43215 We are seeking mission-driven Case Managers to join our integrated community treatment teams. Case Managers provide comprehensive services to adults with severe and persistent mental illness (SPMI) and possible co-occurring substance use disorders. This position is an integral part of an inter-disciplinary team focused on promoting recovery, wellness, and goal attainment. Case Managers work directly with patients to provide community-based supports, including resource development, crisis intervention, treatment planning, and promoting good health. Key Responsibilities Provide community-based services to support patients in managing severe and persistent mental illness. Develop and connect patients to resources that aid in their recovery, such as housing, healthcare, and employment. Advocate for patients’ rights and needs. Promote and encourage healthy lifestyle choices, focusing on both physical and mental health. Offer crisis intervention in emergencies to de-escalate situations and ensure patient safety. Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet goals. Monitor patient progress and adjust treatment plans as necessary to ensure successful goal achievement. Maintain monthly patient contact hours and up-to-date records of patient progress, treatment plans, and interactions. Qualifications Minimum of AA/BA required; LSW/LPC, CCDCII or II strongly preferred. Experience with SPMI and/or SUD population preferred. Strong interpersonal and communication skills. Ability to work effectively as part of an interdisciplinary team. Crisis intervention and de-escalation skills. Knowledge of community resources and healthcare services.
