Overview Physical Therapist – Manor House Category: Physical Therapy & Rehabilitation Position Type: Full-Time Location: 1001 Middleford Road, Seaford, US-DE Responsibilities Provide physical therapy services to residents, including evaluations, planning individual treatment programs and goals, actual treatments, and discharge planning. Follow residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services, building relationships along the way. Make clinically driven, patient-centered treatment decisions that guide all programs and care plans. Collaborate with leadership to provide input into programming, systems, and idea sharing. Work in an in-house program within a welcoming, slower-paced community environment that emphasizes prevention and holistic care. Participate in specialized on-site training delivered during paid work hours; some sessions may be eligible for CEUs. Operate in a setting with state-of-the-art rehab gyms and equipment. Contribute to the team in a non-profit environment, with programs such as student loan forgiveness after 10 years. Discharge planning and related duties as required to support resident care and progression.
