Key Responsibilities

• Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs, goals, and physician direction
• Completes medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Ensures treatment programs are designed to support functional recovery and effective return to community activities within an established length of stay
• Delegates and directs discipline-specific therapy services for assigned patients
• Participates in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adheres to discipline-specific State Practice Act and all applicable regulatory agency requirements, maintaining professional licensure in good standing

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Connecticut
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation in compliance with state, federal, and payer guidelines
• Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards
• Commitment to patient and employee safety practices

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Active involvement in continuing education and professional development activities
