Licensed Wellness Self Management Specialist -Psych Admin - Rivington - Full Time Day

Job Summary:

To be cognizant of the philosophy, standards, objectives and policies of the department and the hospital.

Responsibilities

Clinical/Technical/Service

• Works with program psychiatrist in assessing medical/psychiatric needs of program patients.
• Provides linkage and liaison with primary care provider.
• Provides health evaluation and follow-up to referred patients, management and administration of medication under supervision of MD.
• Evaluates routine urgent health problems and facilitate, coordinate referrals, evaluate laboratory reports.
• Provides health mental education, treatment, rehabilitation and support services to referred patients. Services may be provided in the community as needed.
• Collaborates with program nurse and team members to develop and implement treatment plans involving multiple modalities.
• Participates in team meetings/rounds.
• Documents medical record(s) in a timely and accurate manner.

Organizational/Managerial

• Participates in the departments performance improvement activities.
• Maintains patient/employee confidentiality in the management of information.
• Observes the Health Care System's Compliance policies.

Educational/Professional Development

• Participates in the development of other staff members.
• Meets regulatory, licensure and annual health assessment requirements.
• Identifies learning strengths and needs.
• Utilizes learning resources.

Communication/Relationships

• Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others.
• Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community.

Qualifications

Requirements:

• ASN or BSN required
• Prior nursing experience with the mentally ill with co-morbidities of substance abuse.
• Licensed Registered Nurse in NYS preferred; LPN considered.
• Certification in Psychiatric Nursing and/or Chemical Dependency preferred.
