• Duration: 17 weeks
• Guaranteed Hours: 40 hours per week
• Shift: 8-hour days (08:00 AM – 04:30 PM)
• Weekend Rotation: Once every 6 weeks
• Number of Positions: 2 Travel RNs

Qualifications / Job Requirements

• Minimum 1 year of certified home care / home health nursing experience
• Minimum 1 year of case management experience (discharge planning, care coordination)
• Current Registered Nurse (RN) licensure in the state of Massachusetts
• Reliable, independent means of transportation for home visits (valid driver’s license and vehicle)
• Willingness to work weekends, holidays, and participate in on-call rotations as needed
• Ability to provide clinical services to patients of all ages (pediatrics through geriatrics)
• Strong clinical assessment, documentation (EHR), and interdisciplinary communication skills

Responsibilities

• Provide home health clinical services and home visits to patients from newborns to older adults
• Perform comprehensive home health assessments and develop individualized care plans
• Deliver patient and family education, disease management, and support for care at home
• Manage admissions, coordinate referrals, and perform case management duties in accordance with agency and state healthcare standards
• Maintain accurate, timely electronic documentation in the EHR and communicate effectively with the interdisciplinary care team
• Participate in weekend, holiday, and on-call rotations as required by the travel assignment
