If you're an RN who enjoys leading the clinical picture, making high-level decisions, and setting patients up for success from day one, this role puts you in that seat. As the Admissions Nurse, you are the clinical quarterback, guiding the start of care and ensuring every patient begins with a clear, safe, and effective plan. This is ideal for nurses who want to step into a more strategic, assessment-driven role without moving into management.

What You'll Own:

• Complete Start of Care (SOC) admissions, delivering thorough in-home assessments and accurate OASIS documentation.
• Develop and initiate individualized care plans, ensuring each patient is set up for safe and effective home health services.
• Identify risks, gaps, and service needs early, coordinating the right disciplines and resources from the start.
• Communicate clearly with providers, caregivers, and internal teams to ensure alignment and continuity of care.
• Set the tone for quality, compliance, and patient experience from the very first visit.

What Sets You Apart:

• Active Minnesota RN license with strong clinical foundation.
• Proven experience with home health admissions and OASIS documentation.
• Ability to think critically, prioritize quickly, and make confident decisions independently.
• Strong communication skills with a focus on patient education and care coordination.
• Reliable transportation and comfort working in a field-based environment.
