Senior Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Senior Case Manager to coordinate complex rehabilitation care for adult inpatients. In this role, you will manage a caseload from admission through discharge, developing individualized care plans, facilitating family conferences, and ensuring safe, timely transitions. You will collaborate daily with physicians, rehab nurses, therapists, and social workers to align goals, remove barriers, and secure community resources. The Senior Case Manager also supports utilization review, documentation accuracy, and performance improvement initiatives to drive quality outcomes and patient satisfaction.
