Local/Travel RN Case Manager

Local/Travel RN Case Manager for a 13-week assignment in Bozeman, MT. Responsible for coordinating patient discharges, developing care plans, and managing referral intakes. Earn up to $2,400 weekly with direct deposit and comprehensive benefits.

Key Responsibilities:

• Coordinate patient discharges and follow-up care to ensure seamless transition from hospital to home or facility.
• Assess patient needs and develop comprehensive care plans in collaboration with multidisciplinary teams.
• Manage referral intakes, verify insurance details, and ensure timely processing of authorization requests.
• Utilize Cerner electronic health record system to document patient interactions and care plans accurately.
• Communicate effectively with patients, families, and healthcare providers to facilitate quality care delivery.

Qualifications:

• Valid Montana RN license
• BLS certification
• Minimum 2 years of care coordination or discharge planning experience
• Experience with Medicare, Medicaid, and other insurance types
• Strong bedside manner
• Willingness to commute to Billings, MT
• Ability to manage referral intakes
• Excellent communication skills
