Position Title & Role Overview:

The Nurse Practitioner (NP) plays a crucial role in delivering high-quality, patient-focused primary care. By utilizing our unique Facilitated Virtual Care platform, the NP enhances patient outcomes and minimizes costs for individuals with complex diagnoses and chronic conditions. This position primarily involves virtual consultations, with occasional in-person visits to facilitate care transitions, conduct annual wellness assessments, manage primary care needs, and address urgent situations. The NP works closely with a diverse team to ensure comprehensive care for patients in their homes.

Main Responsibilities:

• Assess, diagnose, and treat patients either in person or through our virtual care system.
• Act as a clinical leader on a daily basis, providing guidance and support to the multidisciplinary team under the supervision of the Medical Director.
• Conduct primary care visits, urgent consultations, and annual wellness exams, ensuring all necessary screenings and quality measures are completed.
• Make appropriate referrals for behavioral health and specialty services.
• Facilitate care transitions to avoid hospital readmissions by collaborating with primary care physicians, hospitalists, and skilled nursing facility providers.
• Participate in the on-call provider rotation to support the 24/7 patient hotline, which requires availability during evenings and weekends on a rotating basis.
• Encourage a collaborative atmosphere among all interdisciplinary team members, contributing to daily huddles, team meetings, and patient discussions.
• Monitor and respond to data collected from remote patient monitoring devices.
• Perform additional duties as assigned.

Knowledge, Skills & Abilities:

• Conduct and document thorough risk assessments to ensure accurate representation of patient conditions.
• Expertise in managing complex, multi-chronic patients, with a proven ability to assess and treat high-acuity cases.
• Experience with Medicare and Medicaid populations, including both inpatient and outpatient or home care environments.
• Preferred proficiency in motivational interviewing, trauma-informed care, and supporting vulnerable groups.
• Strong written, verbal, and digital communication skills, with the ability to engage effectively and proactively with the clinical team.
• Commitment to health equity and improving care for patients in their homes.
• Excellent organizational skills, with the capability to provide clinical leadership and coordination.

Qualifications:

• At least 5 years of experience as a healthcare provider, with a minimum of 3 years in a primary care or related environment.
• Possession of a multi-state Advanced Nursing License or willingness to obtain one.
• A valid driver's license and auto liability insurance.
• Willingness to travel as necessary, although most responsibilities will be conducted virtually.
• Experience in urgent or emergency care settings is advantageous.

California salary range

$140,000—$150,000 USD
