Role Scope We are looking for a dynamic, service-oriented physical therapist who is passionate about supporting elderly persons in their journey. The physical therapist will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. Participant Care Contribute to a center experience that participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong. Exhibit and honor Habitat’s values. Provide physical therapy evaluation and treatment on an acute, restorative, and maintenance level to program participants. Participate in the Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination. Assess condition, create a treatment plan, and discuss implementation of therapy with participants and caregivers. Make home visits as needed and document activities, including participant/caregiver response, in the participant’s chart. Educate participants and family members in proper care, use of exercise programs, and use of durable medical equipment (DME). Document therapy services provided and perform department record-keeping within documentation requirements. Administration and Program Development Continue raising the bar, constructively seek and share feedback, and help implement changes to improve clinical outcomes and participant experience. Assist in ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants. Serve as subject‑matter expert for the development of physical therapy program standardization, evaluation, and team education. Support quality programs and discipline‑specific audits. Maintain quality assurance, safety, environmental, and infection control programs. Perform data collection, prepare reports, and maintain participant statistics. Attend all staff meetings and patient care conferences as scheduled. Required Qualifications Bachelor’s Degree in Physical Therapy Completion of an accredited physical therapy program recognized by APTA, the Committee on Allied Health Education and Accreditation of the AMA, or an equivalent organization approved by CMS Active Physical Therapist license issued by California A minimum of one year experience working with frail or elderly patients Proof of medical clearance for communicable diseases, including a TB test Proof of all immunizations up to date Preferred Qualifications 2+ years as a practicing PT Bilingual: Spanish, Mandarin, or Cantonese preferred Master’s Degree (MSPT) or Doctorate of Physical Therapy (DPT) State‑issued driver’s license, personal transportation, and auto insurance as required by law Location Sacramento, CA (onsite). Compensation Full‑time role eligible for medical, dental, vision insurance, short and long‑term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company‑paid holidays. Expected salary range: $47.00 – $71.00 hourly. Actual offer based on qualifications and experience.
