Licensed Vocational Nurse (LVN) – Congregate Living Health Facility (CLHF)

Job Summary

We are seeking a compassionate, dependable, and clinically skilled Licensed Vocational Nurse (LVN) to provide direct nursing care to medically fragile residents in a Congregate Living Health Facility (CLHF) .

Under the supervision of a Registered Nurse (RN) or physician, the LVN is responsible for delivering safe, high-quality nursing care in accordance with physician orders, individualized care plans, facility policies, and applicable California regulations.

Essential Duties & Responsibilities

• Provide direct nursing care according to physician orders, resident care plans, and facility policies and procedures.
• Administer medications, treatments, enteral feedings, and other prescribed therapies.
• Perform skilled nursing procedures within the LVN scope of practice, including wound care, catheter care, tracheostomy care, and suctioning.
• Monitor residents’ physical, emotional, and clinical status and promptly report significant changes to the RN, physician, and/or Director of Nursing (DON).
• Accurately and timely document assessments, nursing interventions, treatments, medication administration, and resident responses.
• Maintain accurate Medication Administration Records (MARs) and Treatment Administration Records (TARs).
• Assist with resident admissions, transfers, discharges, and physician appointments.
• Communicate professionally and effectively with physicians, pharmacists, families, and members of the interdisciplinary care team.
• Recognize and respond appropriately to emergencies and changes in resident condition.
• Supervise and provide direction to Certified Nursing Assistants (CNAs) and other assigned staff.
• Follow infection prevention and control policies and procedures.
• Participate in resident care planning and interdisciplinary team meetings.
• Maintain resident confidentiality, dignity, privacy, and rights in accordance with HIPAA requirements and facility policies.
• Support compliance with applicable California Title 22 regulations and CLHF licensing requirements.
• Participate in quality assurance and performance improvement activities.

CLHF-Specific Responsibilities

Provide nursing care to residents with complex and medically fragile conditions, including residents who may require:

• Enteral feeding through G-tube/PEG tube
• Care related to neurological conditions and spinal cord injuries
• Respiratory treatments and oxygen therapy

The LVN will closely monitor residents’ respiratory and overall clinical status, immediately report significant changes, and assist with emergency response and resident stabilization until a higher level of medical assistance is available.

Required Qualifications

• Current and active California LVN license in good standing
• Current CPR/BLS certification
• Knowledge of medication administration and skilled nursing procedures
• Ability to work independently while following appropriate clinical supervision and facility protocols
• Ability to recognize changes in resident condition and exercise sound clinical judgment within the LVN scope of practice
• Strong communication, organization, and documentation skills
• Ability to work effectively as part of an interdisciplinary healthcare team

Physical Requirements

Candidates must be able to:

• Stand, walk, bend, and reach throughout the shift
• Assist with lifting, repositioning, and transferring residents using appropriate assistance and equipment
• Respond promptly and safely during emergency situations

Schedule & Compensation

• Shift Length: 10-12-hour shifts
• Compensation: Competitive pay based on experience
