Job Description

Ritemed is seeking a Nurse Practitioner to provide on-site clinical services at Meadowbrook Respiratory & Nursing Center.

This is a facility-based role within a skilled nursing environment. The NP will be responsible for managing post-acute and long-term care residents, working closely with physicians and nursing leadership to drive strong clinical outcomes and reduce hospitalizations.

Key Responsibilities
• Provide comprehensive medical care to residents within the nursing home
• Perform assessments, follow-ups, and manage acute changes in condition
• Collaborate with attending physicians and facility leadership
• Participate in care planning and interdisciplinary meetings
• Ensure timely, accurate documentation
• Support initiatives to improve quality measures and reduce rehospitalizations

Qualifications
• Active New York Nurse Practitioner license
• Minimum of 6 months Nurse Practitioner experience required
• Nursing home or long-term care experience required
• Strong clinical judgment and assessment skills
• Ability to work independently within a structured clinical model
