About the Role
As a Physical Therapist (PT), you will be responsible for designing and implementing individualized rehabilitation plans for our patients, including those with complex medical needs. You will work closely with our interdisciplinary team to ensure that patients receive comprehensive care and support. Your expertise will be essential in helping patients recover from injuries, illnesses, or surgeries, and in managing chronic conditions.
Responsibilities
- Conduct initial and ongoing assessments to determine patient needs and develop customized rehabilitation plans
- Deliver high-quality physical therapy services, including exercises, treatments, and education to patients and their families
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services
- Document patient progress and treatment plans in accordance with facility policies and regulations
- Participate in patient care conferences and contribute to the development of individualized care plans
- Stay up-to-date with the latest research, best practices, and evidence-based therapies to ensure high-quality patient care
