Cancellation Policy: Provider or facility can cancel assignment with minimum 2 week notice with no penalty $1,500 cancellation fee without 2 weeks notice, for both providers and facility. Term with cause - no penalty. Facility can cancel 2 shifts per 13 week contract with no penalty - guaranteed hours only apply at 3rd cancellation. Right to hire with no penalty after 13 week assignment completed. Late Cancellation of Shift by Client Affiliate or HealthTrust: pay subcontractor for the greater of the number of hours worked or 4 hours.
