Our Physical Therapists see patients in the most ideal setting - the patient's home! Our office are located in Dunmore, PA. At Traditional, we do our best to keep each PT's case load within a sensible geographic territory. Shift(s): Full-time/Part-time positions available. Monday through Friday 8:30am to 5:00pm Duties and Responsibilities include, but not limited to the following: Evaluate patients and develop plans of care to achieve desired outcomes Provide therapeutic treatment modalities and implement home therapy / exercise programs Provide instruction to patients to achieve patient goals Maintain communication with ordering physicians and clinical team to promote progress toward goals or seek intervention when needed Participate in quality improvement activities as assigned Timely and accurately document (EMR) all patient assessments, interactions, etc. Requirements: Must possess a current, valid Pennsylvania PT license and have current CPR certification Valid driver's license and ability to travel to case assignments Must be comfortable working in home setting Ability to work independently with minimal supervision and manage time effectively Strong interpersonal and communication skills Why Choose Traditional Home Health Care?
