RN - Registered Nurse Ambulatory Care -Full Time

Position Summary The Journey Clinic RN provides comprehensive nursing care in an ambulatory care setting, assessing, planning, implementing, and evaluating individualized care plans for patients across the lifespan, including direct and telehealth encounters. The RN collaborates with the health care team and maintains compliance with professional nursing standards and regulatory requirements. Compensation and Benefits Sign‑on Bonus: $5,000 for eligible re‑hire and external candidates meeting all payment conditions. Key Responsibilities Facilitate care coordination using the nursing process—assessment, identification, planning, implementation, and evaluation of care and teaching. Provide direct patient care, telephonic or telehealth support, and electronic encounters. Collaborate with interdisciplinary team members. Support clinicians in quality patient care delivery. Maintain professional and regulatory compliance. Qualifications Graduate of an accredited registered nursing school. Associate’s or Bachelor’s Degree in Nursing. Registered Nurse license in the state where care will be provided. Minimum 18 months of professional nursing experience. Proficient in telehealth and electronic documentation.
