Licensed Practical Nurse (LPN) - Itasca, IL
Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Landmark of Itasca, a leading provider of long-term care services in Itasca, IL. As an LPN, you will play a critical role in providing direct patient care to our residents, supervising nursing assistants, and ensuring the highest level of quality care is maintained at all times.
Responsibilities:
Direct patient care to ensure the health, safety, and well-being of our residents
Supervise and mentor nursing assistants, providing guidance and support as needed
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Perform various nursing procedures, including administering medications, wound care, and basic life support
Maintain accurate and detailed records of patient care and communicate with healthcare providers as necessary
Participate in Continuous Quality Improvement (CQI) initiatives to improve patient outcomes and reduce readmissions
Collaborate with other healthcare professionals and team members to deliver comprehensive care
Ensure compliance with facility policies, procedures, and regulatory requirements
Requirements:
Must possess a current, unencumbered LPN license in the state of Illinois
Minimum of 1 year of experience as an LPN, preferred
Possess strong communication and interpersonal skills
Ability to work in a fast-paced environment with frequent interruptions
Commitment to providing high-quality patient care and adhering to facility policies and procedures
Ability to work varied shifts, including evenings, weekends, and holidays
Work Environment:
Work in an office setting and throughout the nursing service area
May be subject to frequent interruptions and hostile or emotional situations
May be required to work beyond normal working hours, including evenings, weekends, and holidays
May be exposed to infectious waste, diseases, and conditions, including TB, AIDS, and Hepatitis B Virus
Physical Requirements:
Ability to move intermittently throughout the work day
Ability to cope with mental and emotional stress of the position
Ability to see and hear with or without aid
Ability to push, pull, move, and/or lift a minimum of thirty-five pounds
