This position includes a mix of in-person and phone triage and support for patients with complex needs. A successful candidate will have a warm and friendly demeanor with patients, be highly organized, confident using EMR systems, and able to think creatively and work well with others. We are looking for a candidate who values building strong relationships with their team of coworkers and patients over the long term. Responsibilities Include Shared responsibility with other members of the nursing team for phone triage and nurse-led face-to-face triage visits during business hours Help patients with complex needs access specialist and community resources Coordinate care for patients after hospital discharge, in collaboration with the nursing team Facilitate our Chronic Care Management Program Participate in quality improvement projects The ideal candidate will have a background in inpatient, primary care, long-term care, or home health nursing. This is a full-time position of 40 hours a week during weekday business hours, with a regular schedule to be established based on candidate and clinic needs.
