Clinical Case Manager: Recovery Support & Advocacy

Hope on Haven Hill in New Hampshire is seeking a dedicated Clinical Case Manager to support pregnant and parenting women in recovery. You will assess client needs, coordinate access to services, and advocate for long-term outcomes within an interdisciplinary team.
