RN - Case Manager

Registered Nurse Case Manager

The position requires a Registered Nurse to serve as a Case Manager. The nurse will work directly with patients to ensure their needs are assessed and met. Responsibilities include discharge planning and creating comprehensive care plans. Knowledge of Epic electronic medical record system is preferred. Candidates must be flexible with scheduling. The role includes a zero-tolerance policy for tardiness and unexcused absences. All candidates must pass a compliance EKG exam. The position may require floating to different units as needed. Compliance deadlines must be met for completion. The nurse will work collaboratively within an interdisciplinary team. Strong communication skills are essential for successful patient advocacy.
