Job Description

Schedule & Hours:

• Monday through Friday, 8:00 AM 5:00 PM
• Caseload expectation: 30 points per week (typically 35 patient visits daily)
• Field-based, with daily travel to patient homes (mileage reimbursement provided)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patient progress, responding to changes and collaborating closely with the interdisciplinary care team.
• Educate patients and families about treatment plans, disease processes, medications, and preventative health.
• Accurately and efficiently document care activities using Home Care Home Base (HCHB).
• Partner with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated service delivery.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Ensure compliance with all regulatory and agency standards for documentation and patient care.

Qualifications:

• Active RN license (current and in good standing) in the state of practice
• Minimum of one year recent home health experience; strong field experience preferred
• Proficiency with Home Care Home Base (HCHB)
• Demonstrated experience with OASIS documentation
• Reliable transportation, current drivers license, and auto insurance

$2,057 - $2,176/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
