Licensed Practical Nurse (LPN)The Licensed Practical Nurse (LPN) is responsible for overseeing and coordinating the healthcare needs of individuals supported by the organization. This position provides direct nursing care, administers medications and treatments, monitors health status, and ensures accurate clinical documentation. The LPN collaborates with physicians, registered nurses, therapists, families, and Direct Support Professionals (DSPs) to develop and implement individualized care plans that promote optimal health outcomes. Responsibilities include staff training, health education, medical appointment coordination, hospital follow-up, emergency response, and advocacy to ensure continuity of care. Monitors individuals' health status through observation, vital sign measurement, symptom tracking, and identification of physical, emotional, and behavioral changes requiring intervention.Clinical documentation and recordkeeping - Provides education to individuals, families, and staff on health-related topics, including medication management, nutrition, hygiene, diabetes care, and preventive health practices.Medical appointment coordination and advocacy - Coordinates and accompanies individuals to medical appointments, facilitating communication with healthcare providers and ensuring appropriate follow-up care.Hospital liaison and continuity of care - Monitors individuals during hospitalizations, advocates for their needs, communicates with treatment teams, and ensures continuity of care throughout transitions.Emergency response and crisis management - Responds promptly and effectively to medical emergencies, applying clinical judgment and established protocols to protect the health and safety of individuals served.Person-centered care and advocacy - While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit; stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
