Registered Nurse (Rn) – Home Health Care
Job Location: Oakland, MI
Job Type: Contract
Job Description:
- Conduct patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures
- Provide wound care, IV therapy, catheter care, and patient education
- Monitor patient progress and communicate changes to physicians
- Coordinate care with therapists, social workers, aides, and other healthcare professionals
- Supervise LPNs and Home Health Aides as required
- Complete accurate and timely documentation in compliance with Medicare, CMS, and agency standards
- Participate in case conferences and quality improvement activities
Requirement:
- Active RN license in good standing
- Minimum 1 year of clinical nursing experience (Home Health preferred)
- OASIS experience preferred
- Current BLS certification
- Valid driver's license and reliable transportation
- Strong assessment, documentation, and communication skills
Preferred Skills:
- Home Health experience
- Case management experience
- Knowledge of Medicare regulations and OASIS documentation
