Your Responsibilities

In your role as a Home Health SLP, you will oversee the assessment and treatment of patients facing speech, language, cognitive, and swallowing challenges:

  • Conduct thorough evaluations of speech, language, cognitive, and swallowing abilities.
  • Create and execute personalized care plans aimed at enhancing communication, cognition, and safety.
  • Deliver therapeutic interventions utilizing evidence-based methods and specialized tools.
  • Evaluate the home setting and suggest adaptive equipment or communication aids as necessary.
  • Educate patients and caregivers on exercises, compensatory strategies, and safety measures.
  • Track patient progress, reassess as required, and modify treatment plans accordingly.
  • Identify any changes in patient condition and proactively communicate with physicians and the care team.
  • Accurately document all care in a timely manner within the electronic medical record (EMR).
  • Collaborate with nurses, therapists, and interdisciplinary teams to ensure cohesive care.
  • Engage in case conferences, discharge planning, and ongoing care coordination.
Qualifications and Requirements
  • Master's or Doctoral degree in Speech-Language Pathology.
  • Current SLP license in the state where you practice.
  • At least 1 year of clinical experience (home health experience preferred).
  • Certificate of Clinical Competence (CCC-SLP) is preferred.
  • Strong clinical judgment and the ability to work independently.
  • Excellent communication and patient education abilities.
  • Comfortable with a field-based role requiring travel between patient homes.
  • Current CPR certification.
  • Valid driver's license, dependable transportation, and auto insurance.
