Position Type: Full-Time | Permanent
Shift: Days | 8-hour shifts
Pay Range: $30.11+/hour (Based on education and experience)
Relocation Assistance: Up to $5,000 available

Job Overview

We are seeking an experienced Care Manager to join an acute care hospital team. This role is ideal for a nursing professional with strong case management experience who is passionate about coordinating patient care, improving outcomes, and ensuring safe, efficient transitions throughout the healthcare journey.

The Care Manager will assess patient needs, coordinate care plans, collaborate with healthcare providers, and support patients and families by connecting them with appropriate resources and services.

Responsibilities

• Coordinate patient care plans with physicians, nurses, and interdisciplinary teams.
• Assess patient needs and determine appropriate levels of care.
• Monitor care delivery to ensure quality, safety, and medical necessity.
• Support discharge planning and care transitions.
• Collaborate with patients, families, and healthcare professionals.
• Identify barriers to care and connect patients with appropriate resources.
• Promote efficient, patient-centered healthcare delivery.

Qualifications

• Associate Degree in Nursing or Diploma from an accredited RN program required
• BSN preferred
• 2+ years of acute care medical-surgical nursing experience required
• Current Kansas RN License required
