Registered Nurse Case Manager Home Health

CarePartners is seeking a compassionate and organized Registered Nurse Case Manager Home Health to join our Healthcare & Medical Services team. In this role, you will provide coordinated, patient-centered nursing care to members in their homes, supporting CarePartners of Connecticut’s mission to improve community health through collaboration, compassion, and high-quality service. As a key clinical partner, you will manage a panel of patients, conduct comprehensive in-home assessments, develop and implement individualized care plans, and coordinate services across our partnered provider network to ensure safe, efficient, and effective care delivery. You will collaborate closely with physicians, social workers, therapists, and other interdisciplinary team members to support timely transitions of care, prevent avoidable hospitalizations, and optimize health outcomes. Responsibilities include performing skilled nursing visits in the home environment, monitoring changes in condition, providing patient and family education on disease management and medications, and advocating for appropriate services and resources. You will document all care in electronic health records using established home health and case management standards, ensuring accuracy, timeliness, and compliance with regulatory and payer requirements. You will also identify gaps in care, address barriers to adherence, and communicate proactively with providers and internal teams to maintain continuity and quality of care. The ideal candidate is a self-directed RN who is comfortable working independently in the field, able to prioritize a dynamic caseload, and committed to compassionate, culturally sensitive care. This position requires strong assessment and critical-thinking skills, excellent communication and interpersonal abilities, and a genuine passion for supporting patients and families in the community.
