Schedule: Rotating shifts with every other weekend and holiday rotation per facility policy Highlights: - Work as a Registered Nurse Case Manager in a 286-bed acute care facility- Manage patient caseload with a ratio of approximately 25:1- Collaborate with interdisciplinary team including RN Case Managers and Social Work Case Managers- Utilize Meditech EMR and Midas case management software systems- Focus on complex discharge planning and care coordination- Float capability to multiple campus locations including psychiatric and rehabilitation departments Requirements: - Active and unencumbered Registered Nurse license in Florida- Prior experience with Midas case management system required- Proficiency with Meditech EMR system required- Case management experience preferred- Strong background in complex discharge planning preferred- Ability to work every other weekend and holidays in rotation with staff Why It's a Great Opportunity: This case management position offers the chance to make a meaningful impact on patient care transitions and outcomes in a multi-campus healthcare system.
