Position Overview

The Non-Medical Post-Hospital Recovery Support Caregiver provides assistance and supervision to clients transitioning home after hospitalization, surgery, or medical treatment. This role focuses on ensuring a safe recovery environment, offering personal support, and helping clients regain independence while following their care plan.

Essential Functions

• Provide personal care assistance (bathing, dressing, grooming, toileting, mobility support) as directed in the care plan.
• Assist with safe transfers, walking, and repositioning to prevent falls and promote healing.
• Monitor and report changes in the client’s physical or mental condition to the supervisor.
• Support medication reminders (non-medical, per care plan).
• Prepare light meals and snacks according to dietary needs and assist with feeding if necessary.
• Help with light housekeeping, laundry, and organization to maintain a clean, safe environment.
• Provide companionship and reassurance during the recovery process.
• Document care provided and communicate with the care team to ensure continuity.
• Respect and maintain client dignity, privacy, and confidentiality at all times.

Skills/Qualifications

• Strong observation skills and ability to recognize changes in condition.
• Knowledge of safe mobility and transfer techniques.
• Patience, compassion, and ability to provide emotional reassurance.
• Good communication skills to interact with clients, families, and the care team.
• Dependability and flexibility to meet varying recovery needs.
• Valid driver’s license and reliable transportation preferred.

Education/Experience Requirements

• CNA, HHA, or equivalent caregiving experience preferred.
• Current certifications in First Aid, CPR, and TB test (or willingness to obtain).
• Successful completion of background check and drug screening, with random checks thereafter.
• Physically able to perform duties, including safe lifting and mobility assistance.
