Homewatch CareGivers of Yorba Linda is seeking an experienced Senior Client Care Manager who is exceptional at building relationships with seniors, family members, caregivers, and healthcare professionals. This is a highly visible, client-facing leadership position for someone with a big heart, excellent instincts, and the confidence to step in and take charge when a situation becomes difficult. The right person will make every client feel like our most important client, while still being able to establish boundaries, have honest conversations, and hold people accountable when necessary.The Client Care Manager is the primary relationship owner for our clients, caregivers, and families. You will spend significant time visiting clients in their homes, understanding their needs, resolving concerns, supporting and training caregivers, and making sure the care we promised is the care we are delivering. This is not primarily a desk position. It requires someone who enjoys being in the field, connecting with people, and responding quickly when circumstances change. Your responsibilities will include:Building strong, trusted relationships with clients, caregivers, and family membersConducting regular in-home client visits, wellness checks, and reassessmentsDeveloping and updating individualized plans of careHelping new clients and families feel comfortable and supported from the beginning of serviceRecognizing changes in a client's condition, environment, or care needs and responding promptlyConducting home-safety reviews, fall-risk assessments, and cognitive screenings within the scope of our non-medical servicesCoordinating care conferences with clients, families, caregivers, and members of our office teamAddressing client concerns with urgency, empathy, sound judgment, and appropriate follow-throughDe-escalating difficult situations while protecting the client relationship and maintaining company standardsCoaching caregivers in the field and reinforcing expectations connected to each client's plan of carePartnering closely with scheduling, operations, recruiting, and leadership to resolve problems before they become larger issuesMaintaining accurate, timely documentation of client visits, changes, and follow-up actionsBringing ideas, concerns, and solutions to our leadership team as we continue to growYou are a natural relationship-builder. People trust you quickly because you listen well, communicate confidently, and make them feel heard. You are warm and compassionate, but you are not a pushover. You know when someone needs reassurance and when a situation requires a direct conversation, a clear boundary, or immediate action. You move with urgency. When a client or family member needs help, you do not let the issue sit in an inbox while everyone waits for someone else to handle it. You are persuasive and comfortable communicating with many different personalities. Whether you call it influence, presence, or simply the gift of gab, you know how to bring people together and move a situation toward a solution. You also understand that great client service is more than being friendly. It requires strong follow-through, attention to detail, accurate documentation, and the ability to coordinate many moving pieces at once.We are especially interested in candidates with experience in:Non-medical home careSenior care or aging servicesAssisted living, memory care, hospice, home health, or another field-based healthcare settingClient care coordination, case management, patient relations, or family supportNursing or another clinical disciplineAn LVN, RN, or other healthcare professional could be an excellent fit, although a nursing license is not required. We are a non-medical home care agency, but healthcare knowledge and sound clinical instincts can be valuable when recognizing changing needs and communicating with families. Spanish-English bilingual skills are a strong plus but are not required.At least 3-5 years of relevant home care, healthcare, senior care, or client-management experienceDirect experience working with clients, patients, seniors, and family membersA proven ability to build relationships and manage difficult or emotional conversationsStrong verbal and written communication skillsExcellent organization, documentation, and follow-throughThe ability to make sound decisions and respond calmly when plans changeConfidence working independently in the field while remaining closely connected to the office teamComfort speaking up, contributing ideas, and participating as a member of the leadership teamA valid driver's license, reliable vehicle, and current automobile insuranceThe ability to travel regularly between client homes throughout our service areaHome care is a 24-hour business, even though this is primarily a weekday position. This is a full-time role with a regular Monday-through-Friday schedule. It also includes:Occasional weekend or after-hours involvement when a client situation requires leadership supportParticipation in a shared emergency on-call rotation with approximately four to five members of the office teamRegular travel to client homes throughout the workdayThe on-call responsibility is shared. This is not a position in which one person is expected to carry the phone every night or every weekend.
