In-Home NP/PA (Part Time) - Denver, CO - Jefferson County

Job Highlights The visit lasts about 45 minutes, during which the clinician connects one‑on‑one, answers health questions, reviews medication and medical history, performs a physical evaluation, and, if ordered by the person's health plan, conducts additional diagnostic tests (e.g., diabetic eye exams, spirometry). The clinician uses an iPad and an intuitive clinical workflow. Prescribing medicine, ordering lab tests, or altering current treatment regimens are not required. In this role, you will: Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Help reach millions of people nationwide by providing connections to the right care for unique needs. Work with Signify Health as a clinician colleague, fulfilling the responsibilities of the role. Be flexible to travel locally and within licensed states as needed. Conduct virtual visits as required. Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams. Environment and Safety As an in‑home clinician, your primary workplace is the private residence of health plan members. You may encounter household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to safely perform duties in these environments. Required & Preferred Qualifications (NPs and PAs) Active, unrestricted license(s) in all coverage areas. Board certification (required). Multi‑state licenses (preferred).
