About the Role:
As a Registered Nurse with VNA Health Group's Hospice program, you will be responsible for providing skilled nursing care to patients in their place of residence, including their homes, assisted living facilities, and skilled nursing facilities. You will work in collaboration with a multidisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of our patients. This is a full-time opportunity that offers a rewarding and challenging experience for a dedicated nurse.
Responsibilities:

Assess patients' physical and emotional needs, develop and implement individualized care plans, and provide skilled nursing care in accordance with established standards and regulations
Collaborate with the interdisciplinary team to ensure comprehensive care, including physicians, social workers, home health aides, and spiritual care providers
Provide emotional support and bereavement counseling to patients and their families
Document patient care and interactions accurately and thoroughly in accordance with established policies and procedures
Participate in quality improvement initiatives and utilize data to drive improvements in patient care
